The Pos-T-Vac Ultimate II Round is Timm Medical's standard tension ring — the ring included with the MVP 700 and Automatic 2000 VED kits, and available individually for reorder or to find a size outside those included in the original kit. The circular center of the Ultimate II provides even, lasting tension around the base of the penis to maintain erection after vacuum therapy, for up to 30 minutes per session. The silicone and gel blend gives the ring a soft, pliable feel that is comfortable during extended wear while maintaining consistent constriction tension. Looped handles on each ring allow the user to guide it over the loading cone and onto the cylinder with straightforward finger placement — no specialized tool or technique needed. Each ring provides approximately 20–30 uses before the material tension changes enough to warrant replacement. Choosing the Right Size — How Tension Ring Sizing Works

Tension ring sizing is not as straightforward as body measurements alone. The correct size for any individual depends on two factors together: the inside diameter of the ring (which determines how snugly it fits around the base of the penis) and the tension level of the ring material at that size (which determines how effectively it restricts venous outflow to maintain the erection). A ring that fits comfortably in diameter but has insufficient tension will not maintain the erection adequately. A ring that has appropriate tension but is too small in diameter will cause discomfort or be difficult to remove.

Timm Medical's guidance: finding the best size is largely a process of trial. The three middle sizes (RND01, RND02, RND03) are included with most Pos-T-Vac VED kits and cover the most commonly needed range. If you consistently find that the smallest ring included (RND01) is still too large or doesn't provide enough tension, try the X-Small (RNDX). If the largest ring included (RND03) is too tight or uncomfortable, try the X-Large (RND04).

Starting point by anatomy: men with a smaller base circumference typically start with sizes X or 1; men with a medium base circumference typically start with size 2; men with a larger base circumference typically start with sizes 3 or 4. These are starting points only — the effective size for a given individual depends on the combination of fit and tension, not measurement alone.

When to Replace Tension Rings

Each Ultimate II Round Ring provides approximately 20–30 uses. Replace when:

• The erection is not maintained as effectively as when the ring was new
• The ring feels less firm or elastic than when first used
• The ring shows visible wear, stretching, or cracking
• The ring was exposed to oil-based lubricant, which degrades the silicone material
